Reporting Materials

Reporting materials are available in Microsoft Office and Adobe Reader (PDF) formats. Microsoft Office documents required for submission must be completed electronically, printed, and submitted. Adobe Reader (PDF) documents required for submission must be printed, completed by hand, and submitted.

Please allow 20 working days for a check to be created once reimbursement request has been recieved by ADFP Trust Fund staff.
